The entire life cycle of exosomes, from early endosome development to mature multi cyst formation to exosome transport and uptake, requires the involvement of large amounts of lipids such as cholesterol, phospholipids, and sphingolipids, which are vital substances for exosome phospholipid bilayer formation. Compared to donor cells, exosomes are more enriched in sphingolipids, cholesterol, and phosphatidylserine. The lipid composition of the membrane structure of exosomes from different sources is selective.

The lipid composition of exosomes is like that of lipid rafts, and exosomes have a higher lipid content than other extracellular vesicles and a higher stability to detergents. Exosomal lipid molecules can interact with receptors such as G proteins and lipid molecules on the recipient cell membrane, which regulates signaling pathways and biological activities within the recipient cell. Exosomal lipidomic analysis can be used to identify novel diagnostic markers and exosome bioactivity analysis. The lipidomic analysis is characterized by high lipid homology, difficult identification, and high sample purity requirements. The exosomal lipidomic analysis also has the general problem of a small sample size, which is challenging to accurately characterize and quantify exosomal lipids.
